56.5.3 Summons
1. If you receive notice that the respondent has failed to attend an
examination or treatment, deliver the case file to the judge for direction
on issuing a summons. KRS 222.435.
2. If a summons is ordered, use AOC Form 702A, Summons Involuntary
Treatment (Substance Use Disorder) , and complete a summons screen.
Use summons type IT. Give five (5) copies of the summons; petition; AOC Form 702A
and order appointing counsel, setting hearing and appointing Summons type IT
physician/QHP to the Peace Officer for distribution as follows:
• Respondent
• Officer’s File
• Officer’s Return
• One of each for the Qualified Health Professionals (attach to each of
AOC Form 703A
the two copies a blank copy of AOC Form 703A, Certification of
Qualified Health Professional (Involuntary Treatment-Substance
Use Disorder).”
NOTE: Only send AOC Form 703A to QHP if examination boxes are
checked on summons.
File, date, initial
3. File return of summons by applying the FILED stamp; add date and
your initials. Enter the return information on the summons screen. FILE, date, initial
4. Certification of Physician/QHP. When the certification is received, apply Document type CIT
the FILED stamp to the certification; add the date and your initials. File
certification by completing a document screen. Use document type CIT.
