56.2.5 Involuntary Commitment Hearing
A. Respondent DOES NOT Meet Criteria for
Quick Reference
Involuntary Commitment AOC Form 708.4
1. If, on AOC Form 708.4, Judgement and Order of Involuntary Enter, date, initial
Commitment of Respondent or Dismissal & Release of Respondent, DOC Type JODR
the court or the jury finds that the Commonwealth did not establish
beyond a reasonable doubt that Respondent meets the criteria for
involuntary commitment, apply the ENTERED stamp; add the date
and your initials. Enter by completing a document screen. Use
document type JODR. Notice of Entry
Give notice of entry by mailing to:
• Commonwealth's Attorney
• Respondent / Respondent’s Attorney
• Guardian ad Litem
• Forensic Psychiatric Facility Close case screen
2. Close the case by completing the case screen with disposition date,
disposition type DIS, and judge at disposition.
B. Respondent Meets the Criteria for Involuntary Commitment
1. If, on AOC Form 708.4, Judgement and Order of Involuntary Quick Reference
Commitment of Respondent or Dismissal & Release of Respondent, Notice of Entry
the court or the jury finds that the Commonwealth did establish Include two blank
beyond a reasonable doubt that Respondent meets the criteria for copies of AOC Form
involuntary commitment, apply the ENTERED stamp; add the date 708.3 for QMHPs
and your initials. Enter by completing a document screen. Use
document type JOIC.

Give notice of entry by mailing to:
• Commonwealth's Attorney
• Respondent / Respondent’s Attorney
• Guardian ad Litem
• Forensic Psychiatric Facility (include two additional copies for
Physician and QMHP, and attach to each a blank AOC Form
Close Case Screen
708.3, Examination Certification for Involuntary Commitment)
