5.2 Judgment/Order
1. When a signed order is received from the judge, apply the ENTERED Enter, date, initial
stamp; add the date and your initials.
2. Enter the order on the document screen. Use document type ADDCE. Document type
Serve notice of entry on the parties by making a copy of the order that has ADDCE
been stamped "ENTERED" and mail or hand deliver it to the parties or
attorney. The method of service and date of service must be completed
NOE
on the document screen. See Notice of Entry.

3. If the order directs the Registrar of Vital Statistics to issue a death
and Family Services
certificate at no charge, mail a certified copy of the Order to CHFS, Vital
Registrar of Vital

 A party receiving periodic payments of damages for personal injuries established by
settlement or judgment in resolution of a tort claim must seek court approval to transfer the
structured settlement payment rights.
 An application for approval of the transfer of structured settlement payment rights may be
filed in the county where the applicant resides or in the county where the original action
was or could have been filed.
