30.10.1 Court Action for Designation of Standby
Guardian/Conservator
Quick Reference
1. Upon receipt of AOC Form 777, Verified Petition for the Voluntary AOC Form 777
Appointment of a Guardian or Conservator and Standby Guardian and Enter, date, initial
Order, apply the ENTERED stamp; add the date and your initials.
2. Enter on a document screen using the appropriate document type. Document type
• If the petitioner is a guardian, use OAG * If guardian - OAG
* If conservator -
• If the petitioner is a conservator, use OCON OCON
3. Give notice of entry to the attorneys of record and all parties not
represented by counsel. Include notice of entry date and method. For
more information, see Notice of Entry.
4. If the order designates the applicant as standby guardian or conservator, Give notice of entry
the applicant has 10 days following death, resignation, removal or
incapacity of the guardian or conservator to accept the designation as
standby guardian or conservator.
5. Upon receipt of an acceptance of the designation of standby guardian or
conservator, apply the FILED stamp; add the date and your initials. File, date, initial

7. Serve a copy of the acceptance on the ward by mailing a copy to the ward
and his/her nearest adult relative.
