30.10.2 Appointment of Standby Guardian or Conservator
1. Upon receipt of AOC Form 770, Order of Appointment of Conservator or Quick Reference
AOC Form 775, Order of Appointment of Guardian and AOC Form 825, AOC Form 770
Fiduciary Bond (KRS 395.130, 62.060), collect the bond filing fee (if any AOC Form 775
bond is required) as set forth in the Accounting Manual and give a receipt. AOC Form 825
NOTE: Clerk must attest to the fiduciary/surety identity and signature Collect bond filing
