30.10 Designation of Standby Guardian or Conservator
1. Upon receipt of AOC Form 777, Verified Petition for the Voluntary
AOC Form 777
Appointment of a Guardian or Conservator and Standby Guardian and
File, date, initial
Order KRS 387.330, apply the FILED stamp; add the date and your
initials.
Document types
2. File the application by completing a document screen using the following * PVA
document types: * If guardian - OAG
* If conservator -
• PVA
OCON
• If the petitioner is a guardian, use OAG
• If the petitioner is a conservator, use OCON
3. Do not issue a summons. Do not issue
summons
4. Deliver the case file to the judge. If the judge directs, schedule a hearing
and give notice. Complete a scheduled events screen for the date of the
hearing.
