a)     The name, address and telephone number of the applicant;
                    b)     The proposed location of the farm stand on the private property;
                    c)     If the applicant is not the owner of the property, the written permission from
                           the owner of the property along with the owner’s name, address and
                           telephone number; and
                    d)     Submittal of soil testing.
