(1) a dated receipt, signed by the owner/operator of the licensed solid
            waste disposal facility where the debris was deposited.
                 (2) the receipt shall contain a description of the debris disposed of, and its
            weight, or volume.
                 (3) the permit holder shall also provide the building commissioner with an
            affidavit that the receipt submitted is true and accurate to the best of the permit
            holder's knowledge.
                 (4) if the permit holder cannot dispose of the debris at the location
            indicated, it shall be the permit holder's obligation to obtain an amendment to
            the permit reflecting the new disposal location. The building commissioner
            shall be so notified, and the permit amended, prior to the disposal of the debris
            at the new disposal location.
