The director shall refer any person found in violation of a prohibited discriminatory
 practice or retaliation based on source of income more than once within a 12-month
 period to the director of the health department and recommend that the person's rental
 permits be placed on special probationary status pursuant to section 34-855 (6) of the City's Code of Ordinances pending the completion of a corrective action
 plan prescribed by the director. (c)
