D.
In the absence of written consent, the consent required by paragraph C is considered to have been granted to an operator upon the operator's delivery to the owner, in person or by certified mail, return receipt requested by the addressee, the following:
(1)
A copy of this section;
(2)
A signed statement that the operator will be bound by the terms of this section to the owner of the property upon which the cable television system facilities or over-the-air reception device is to be affixed; and
(3)
Notice to the owner in clear, understandable language that describes the owner's rights and responsibilities.
